The Dyslexia tool records student Dyslexia information as well as district-defined fields. The list of dyslexia records is sorted by Start Date.

A Dyslexic record does NOT indicate the student is dyslexic. Students with Dyslexia records may be: Dyslexic, At Risk, or Not Screened.

Start Date

Use the Start Date to indicate the effective date for the student's dyslexia record. This is a required field.

Dyslexia Risk

The Dyslexia Risk code indicates the results of required screening for dyslexia and related disorders.

Code	Description	Definition
01	Screened and Not At Risk	Screened and determined to be not at risk for dyslexia or related disorders
02	Screened and At Risk	Screened and determined to be at risk for dyslexia or related disorders
03	Not Screened	Not screened for dyslexia or related disorders

Dyslexia Services

This field indicates the type of dyslexia or related service a student has received at any time during the school year.

Code	Description	Definition
00	No Services	Does not receive services for dyslexia or a related disorder.
01	Receives Services	Receiving services for dyslexia or a related disorder with an individualized education program developed for the student.
02	Receives Specialized Instruction	Receiving instruction that meets applicable dyslexia program criteria established by the State Board of Education; and is provided by a person with specific training in providing that instruction.
03	Allowed Modifications or Accommodations	The student is permitted, on the basis of having dyslexia or a related disorder, to use modifications in the classroom or accommodations in the administration of assessment instruments.

Dyslexia Screening Exception Reason

Use the drop-list to indicate the reason a student was not screened for dyslexia.

Code	Description
01	Grade 1 student withdrew from LEA on or before Jan 31st.
02	Kindergarten student withdrew from LEA on or before the last instructional day of the year.
03	Grade 1 student enrolled in the LEA after January 31st.
04	Student currently identified and receives dyslexia services.
05	Dyslexia screening inappropriate for the child.
06	Dyslexia screening included in Special Education evaluation or 504 evaluation process.
07	Parent or child repeated refusal of dyslexia screening.
08	Student absent during the designated dyslexia screening, no make-up provided during screening window.
09	No appropriately trained and/or qualified individual in the LEA, to conduct dyslexia screening.
10	Technology access or failure prevented the LEA from screening the student for dyslexia.
11	No screening instrument available or no screening instrument adopted by district-level committee.
12	Other
