The Blue Card Requirements tool lists the student's status of vaccine requirements for Pre-Kindergarten, TK/K-12, and 7th Grade. Information entered here is reported on the Blue Card Report 2019. The Blue Card Report can be printed from this tool.
Field | Description |

Student Immunization Summary The date/timestamp is when the last nightly process was run to check for compliance. See the California Immunization article for more guidance on student immunization compliance rules. | |
Vaccine Code | The immunization code abbreviation. |
Compliance Status | The immunization compliance status. |
Exemption Code | The exemption code abbreviation. |
Follow-up Date | The date when immunization follow up will happen. |
Kindergarten | |
Status of Requirements | The status of the student's immunization compliance for kindergarten. Options include:

Date Requirements Met | The date the immunization compliance rules were met. |
Verified By | The person who checked the Kindergarten immunization compliance for the student. |
Staff Initials | The initials of the person who checked the Kindergarten immunization compliance for the student. |
Follow-up Date Only displays if 02: Requires Follow-up is selected for the Status of Requirements field | The date the immunization follow up will occur. |
TK/K-12 | |
Status of Requirements | The status of the student's immunization compliance for TK/K-12. Options include:

Verified By | The person who checked the TK/K-12 immunization compliance for the student. |
Staff Initials | The initials of the person who checked the TK/K-12 immunization compliance for the student. |

7th Grade | |
Status of Requirements | The status of the student's immunization compliance for 7th grade. Options include:

Staff Initials | The initials of the person who checked the 7th grade immunization compliance for the student. |

Report Logic
Review the student's immunization record for admission to: Pre-K (Childcare of Preschool, TK/K-12, or advancement to 7th grade to complete the appropriate row in the Status of Requirements section.
Enter Blue Card Requirements
There are three sections on this tool:
- Pre-Kindergarten
- TK/K-12
- 7th Grade
Enter information for each section:
- Select the Requirement Status.
- If 01: Has All Required Doses is selected, enter a date in the Date Requirements Met field.
- If 02: Requires Follow-up is selected, mark Temporary Medical Exemption, Missing Doses In Progress, or Missing Doses Overdue AND enter a Follow-up Date.
- If 03: Other is selected, mark one of the other options.
- Enter the staff member name in the Verified By field.
- Enter the Staff Initials.
Click Save when finished.
