Combined Incident Reporting gathers information related to violations of school policies, state or federal statutes or regulations. Four behavior reports collect information about behavior incidents, offenders, actions and victims.

Combined Incident Reporting Editor

Report Logic

Reports all Person information where the following are true:

  • The related incident date is on or after the first instructional day of the selected calendar.
  • The related incident date is on or before the Effective Date (from the extract editor).
  • Role is O1, O2, O3, O4, or O5.

 Students are NOT included if 

  • their enrollment record is marked as State Exclude;
  • their enrollment record is marked as No Show;
  • their Grade Level of enrollment is marked as State Exclude; or
  • their Calendar of enrollment is marked as State Exclude.

A number indicating the offender's role in the incident.

  • O1 - Offender:Student attending this school
  • O2 - Offender:Student from another school
  • O3 - Offender:School Personnel
  • O4 - Offender:Other Adult
  • O5 - Offender:Unknown
  • P - Participant
  • W - Witness

 Perp_Injury 

A number indicating what level the injury was sustained by the offender.* 1 - No Injury

  • 2 - Minor Injury

 Perp_IEP 

A letter indicating whether the offender had an IEP: Y = Yes, N = No.
