Health vaccines and their associated compliance listings are based on state guidelines. While Infinite Campus maintains standard guidelines for all states, districts have the ability to enter additional compliances. The following information describes vaccine compliance rules as defined by the Utah Department of Health.

DTaP, DTP

Diphtheria-tetanus-pertussis, combined [DTaP, DTP] (code: DTaP-DTP)

Rule	Requirements
Age 0-1 months No Requirement	Students 1 month old and younger are not required to receive any doses of the DTaP, DTP vaccine.
1 dose Age 2-5 months Compliant	Students are considered compliant if the following conditions have been met: the student is at least 2 months old, but no older than 5 months old, the student receives one dose of the DTaP/DTP, DT, TD, or Tdap vaccine, and the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance.
2 doses Age 4-8 months Compliant	Students are considered compliant if the following conditions have been met: the student is at least 4 months old, but no older than 8 months old, the student receives two doses of the DTaP/DTP, DT, TD, or Tdap vaccine, the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance, and the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.
3 doses Age 6-18 months Compliant	Students are considered compliant if the following conditions have been met: the student is at least 6 months old, but no older than 18 months old, the student receives three doses of the DTaP/DTP, DT, TD, or Tdap vaccine, the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, and the difference between the third and previous dose is greater than 28 days with a 4 day tolerance.
4 doses under 7 Compliant	Students are considered compliant if the following conditions have been met: the student is no older than 83 months, the student is in state grade: PK, the student receives four doses of the DTaP/DTP, DT, TD, or Tdap vaccine, the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, the difference between the third and previous dose is greater than 28 days with a 4 day tolerance, the difference between the fourth and previous dose is greater than 4 months with a 4 day tolerance, and the difference between the fourth dose and the student's birthdate is greater than 12 months with a 4 day tolerance.
4 doses Compliant	Students are considered compliant if the following conditions have been met: the student receives four doses of the DTaP/DTP, DT, TD, or Tdap vaccine, the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, the difference between the third and previous dose is greater than 28 days with a 4 day tolerance, the difference between the fourth and previous dose is greater than 4 months with a 4 day tolerance, and the difference between the last dose and the student's birthdate is greater than 48 months with a 4 day tolerance.
5 doses Compliant	Students are considered compliant if the following conditions have been met: the student receives five doses of the DTaP/DTP, DT, TD, or Tdap vaccine, the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, the difference between the third and previous dose is greater than 28 days with a 4 day tolerance, the difference between the fourth and previous dose is greater than 4 months with a 4 day tolerance, and the difference between the fifth and previous dose is greater than 6 months with a 4 day tolerance.
3 doses 7+ Compliant	Students are considered compliant if the following conditions have been met: the student is at least 84 months (7 years) old, the student receives three doses of the DTaP/DTP, DT, TD, or Tdap vaccine, the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance, the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, the difference between the third and previous dose is greater than 28 days with a 4 day tolerance, and the difference between the last dose and the student's birthdate is greater than 84 months with a 4 day tolerance.
4 doses 7+ Compliant	Students are considered compliant if the following conditions have been met: the student is at least 84 months (7 years) old, the student receives four doses of the DTaP/DTP, DT, TD, or Tdap vaccine, the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance, the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, the difference between the third and previous dose is greater than 28 days with a 4 day tolerance, the difference between the fourth and previous dose is greater than 6 months with a 4 day tolerance, and the difference between the last dose and the student's birthdate is greater than 84 months with a 4 day tolerance.
3 doses In Progress	Students are considered in progress if the following conditions have been met: the student receives three doses of the DTaP/DTP, DT, TD, or Tdap vaccine, and the difference between the last dose and the effective date is less than 6 months with a 1 day tolerance.
2 doses 7+ In Progress	Students are considered in progress if the following conditions have been met: the student is at least 84 months (7 years) old, the student receives two doses of the DTaP/DTP, DT, TD, or Tdap vaccine, and the difference between the last dose and the effective date is less than 6 months with a 1 day tolerance.
1 dose In Progress	Students are considered in progress if the following conditions have been met: the student receives one dose of the DTaP/DTP, DT, TD, or Tdap vaccine, and the difference between the last dose and effective date is less than 28 days with a 0 (zero) day tolerance.

Tdap

Tetanus, Diphtheria and Acellular Pertussis [Tdap] (code: Tdap)

Rule	Requirement
1 dose grades 7-12 Compliant	Students are considered compliant if the following conditions have been met: the student is in state grade: 07, 08, 09, 10, 11, 12, the student receives one dose of the Tdap vaccine, and the difference between the last dose and the student's birthdate is greater than 120 months with a 4 day tolerance.
No dose grades 7-12 Non-compliant	Students are considered non-compliant if the following conditions have been met: the student is in state grade: 07, 08, 09, 10, 11, 12, and the student receives no doses of the Tdap vaccine.

Polio

Rule	Requirement
Age 0-1 months No Requirement	Students 1 month old and younger are not required to receive any doses of the Polio vaccine.
Age 18+ No Requirement	Students 18 years old and older are not required to receive any doses of the Polio vaccine.
1 dose Age 2-3 months Compliant	Students are considered compliant if the following conditions have been met: the student is at least 2 months old, but no older than 3 months old, the student receives one dose of the IPV or combined IPV/OPV Polio vaccine, and the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance.
2 doses Age 4-5 months Compliant	Students are considered compliant if the following conditions have been met: the student is at least 4 months old, but no older than 5 months old, the student receives two doses of the IPV or combined IPV/OPV Polio vaccine, the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, and the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.
4 doses before 08/07/2010 Compliant	Students are considered compliant if the following conditions have been met: the student receives four doses of the IPV or combined IPV/OPV Polio vaccine, the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, the difference between the third and previous dose is greater than 28 days with a 4 day tolerance, the difference between the fourth and previous dose is greater than 28 days with a 4 day tolerance, and the student receives the last dose before 08/07/2010.
4 doses Compliant	Students are considered compliant if the following conditions have been met: the student receives four doses of the IPV or combined IPV/OPV Polio vaccine, the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, the difference between the third and previous dose is greater than 28 days with a 4 day tolerance, the difference between the fourth and previous dose is greater than 6 months with a 4 day tolerance, and the difference between the fourth dose and the student's birthdate is greater than 48 months with a 4 day tolerance.
3 doses Compliant	Students are considered compliant if the following conditions have been met: the student is at least 6 months, but no older than 47 months old, the student receives three doses of the IPV or combined IPV/OPV Polio vaccine, the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, and the difference between the third and previous dose is greater than 28 days with a 4 day tolerance.
3 doses 4 yrs Compliant	Students are considered compliant if the following conditions have been met: the student is at least 48 months (4 years) old, the student receives three doses of the IPV or combined IPV/OPV Polio vaccine, the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, the difference between the third and previous dose is greater than 6 months with a 4 day tolerance, and the difference between the third dose and the student's birthdate is greater than 48 months with a 4 day tolerance.
1 dose In Progress	Students are considered in progress if the following conditions have been met: the student receives one dose of the IPV or combined IPV/OPV Polio vaccine, and the difference between the last dose and the effective date is less than 28 days with a 0 (zero) day tolerance.
2 doses In Progress	Students are considered in progress if the following conditions have been met: the student receives two doses of the IPV or combined IPV/OPV Polio vaccine, and the difference between the last dose and the effective date is less than 6 months with a 1 day tolerance.

Measles Mumps Rubella MMR

Rule	Requirements
Age 0-11 months No Requirement	Students 11 month old and younger are not required to receive any doses of the MMR vaccine.
1 dose PK Compliant	Students are considered compliant if the following conditions have been met: the student is in state grade: PK: Pre-kindergarten, the student receives one dose of the MMR or MMRV vaccine, and the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance.
1 dose 12-59 months M, M, R Compliant	Students are considered compliant if the following conditions have been met: the student is in state grade: PK: Pre-kindergarten, the student receives one dose of the Measles, MMR, or MMRV vaccine, and the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, OR the student receives one dose of the Mumps, MMR, or MMRV vaccine, and the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, OR the student receives one dose of the Rubella, MMR, or MMRV vaccine, and the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance.
2 doses K-12 Compliant	Students are considered compliant if the following conditions have been met: the student is in state grade: 00: Kindergarten, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12, the student receives two doses of the MMR or MMRV vaccine, the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance.
2 doses K-12 Compliant	Students are considered compliant if the following conditions have been met: the student is in state grade: 00: Kindergarten, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12, the student receives two doses of the Measles, MMR, or MMRV vaccine, the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, OR the student receives two doses of the Mumps, MMR, or MMRV vaccine, the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, OR the student receives two doses of the Rubella, MMR, or MMRV vaccine, the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance.
1 dose In Progress	Students are considered in progress if the following conditions have been met: the student receives one dose of the Measles, Mumps, Rubella, MMR, or MMRV vaccine, and the difference between the first dose and the effective date is less than 28 days with a 0 (zero) day tolerance.

Hepatitis B

Rule	Requirements
1 dose 0-1 months Compliant	Students are considered compliant if the following conditions have been met: the student is at least 1 month old or younger, and the student receives one dose of the Hepatitis B vaccine.
2 doses 2-3 months Compliant	Students are considered compliant if the following conditions have been met: the student is at least 2 months, but no older than 3 months old, the student receives two doses of the Hepatitis B vaccine, and the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.
3 doses Compliant	Students are considered compliant if the following conditions have been met: the student receives three doses of the Hepatitis B vaccine, the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, the difference between the last and the second valid dose is greater than 56 days with a 4 day tolerance, the difference between the last and the first valid dose is greater than 112 days with a 4 day tolerance, and the difference between the last dose and the student's birthdate is greater than 168 days with a 4 day tolerance.
2 doses Hep B-2 Over 11 yrs Compliant	Students are considered compliant if the following conditions have been met: the student is at least 132 months (11 years) old, the student receives two doses of the Hepatitis B - 2 dose vaccine, and the difference between the second dose and the previous is greater than 4 months with a 4 day tolerance.
1 dose Adult Hep B-2 Over 11 yrs In Progress	Students are considered in progress if the following conditions have been met: the student is at least 132 months (11 years) old, the student receives one dose of the Hepatitis B - 2 dose vaccine, and the difference between the last dose and the effective date is less than 4 months with a 0 (zero) day tolerance.
2 doses In Progress	Students are considered in progress if the following conditions have been met: the student receives two doses of the Hepatitis B vaccine, and the difference between the last dose and the effective date is less than 56 days with a 0 (zero) day tolerance.
1 dose In Progress	Students are considered in progress if the following conditions have been met: the student receives one dose of the Hepatitis B vaccine, and the difference between the last dose and the effective date is less than 28 days with a 0 (zero) day tolerance.

Varicella

Rule	Requirements
Age 0-11 months No Requirements	Students 11 month old and younger are not required to receive any doses of the Varicella vaccine.
2 doses Compliant	Students are considered compliant if the following conditions have been met: the student is in state grade: 00: Kindergarten, 01, 02, 03, 04, 05, 07, 08, 09, 10, 11, 12, the student receives two doses of the MMRV or Varicella vaccine, the difference between the first dose and the students birthdate is greater than 12 months with a 4 day tolerance, and the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.
1 dose Compliant	Students are considered compliant if the following conditions have been met: the student is in state grade: PK: Pre-Kindergarten, 06 the student receives one dose of the MMRV or Varicella vaccine, and the difference between the first dose and the students birthdate is greater than 12 months with a 4 day tolerance.
1 dose 13yo In Progress	Students are considered in progress if the following conditions have been met: the student receives one dose of the MMRV or Varicella vaccine, and the difference between the last dose and the student's birthdate is less than 28 days with a 0 (zero) day tolerance.
1 dose < 13yo In Progress	Students are considered in progress if the following conditions have been met: the student receives one dose of the MMRV or Varicella vaccine, and the difference between the last dose and the student's birthdate is less than 3 months with a 1 day tolerance.

Meningococcal MCV4

Rule	Requirements
1 dose Grades 7-12 Compliant	Students are considered compliant if the following conditions have been met: the student is in state grade: 07, 08, 09, 10, 11, 12, the student receives one dose of the Meningococcal vaccine, and the difference between the last dose and the student's birthdate is greater than 120 months with a 4 day tolerance.
No dose Grades 7-12 Non-Compliant	Students are considered non-compliant if the following conditions have been met: the student is in state grade: 07, 08, 09, 10, 11, 12, and the student does not receive any doses of the Meningococcal vaccine.

Hib

Haemophilus influenza, type B [Hib] (code: Hib)

Rule	Requirements
Age 0-1 months No Requirement	Students 1 month old and younger are not required to receive any doses of the Hib vaccine.
Age 5 years and older No Requirement	Students 60 months (5 years) old and older are not required to receive any doses of the Hib vaccine.
1 dose Age 2-3 months Compliant	Students are considered compliant if the following conditions have been met: the student is as least 2 months, but no older than 3 months old, the student receives one dose of the Hib or the Hib 3 dose vaccine, and the difference between the last dose and the student's birthdate is greater than 42 days with a 4 day tolerance.
2 doses Age 4-5 months Compliant	Students are considered compliant if the following conditions have been met: the student is as least 4 months, but no older than 5 months old, the student receives two doses of the Hib or the Hib 3 dose vaccine, the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, and the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.

1 dose 15 months 1st shot Compliant	Students are considered compliant if the following conditions have been met: the student is as least 15 months old, the student receives one dose of the Hib or Hib 3 dose vaccine, and the difference between the first dose and the student's birthdate is greater than 15 months with a 4 day tolerance.
1 dose In Progress before 1 yr In-Progress	Students are considered in progress if the following conditions have been met: the student receives one dose of the Hib or Hib 3 dose vaccine, the difference between the last dose and the effective date is less than 28 days with a 0 (zero) day tolerance, and the difference between the last dose and the student's birthdate is less than 11 months with a 0 (zero) day tolerance.
1 dose in Progress after 1 yr In-Progress	Students are considered in progress if the following conditions have been met: the student is at least 12 months old, the student receives one dose of the Hib or Hib 3 dose vaccine, the difference between the last dose and the effective date is less than 56 days with a 0 (zero) day tolerance, and the difference between the last dose and the student's birthdate is greater than 12 months with a 0 (zero) day tolerance.
2 doses in Progress In-Progress	Students are considered in progress if the following conditions have been met: the student is as least 7 months old, the student receives two doses of the Hib vaccine, and the difference between the last dose and the effective date is less than 56 days with a 0 (zero) day tolerance.
2 doses Hib 3 dose in Progress before 1 year In-Progress	Students are considered in progress if the following conditions have been met: the student is as least 7 months, but no older than 12 months olf, the student receives two doses of the Hib 3 dose vaccine, and the difference between the last dose and the effective date is less than 56 days with a 0 (zero) day tolerance.

Hepatitis A

Rule	Requirements
2 doses Compliant	Students are considered compliant if the following conditions have been met: the student receives two doses of the Hepatitis A vaccine, the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and the difference between the second and previous dose is greater than 6 months with a 4 day tolerance.
1 dose In Progress	Students are considered in progress if the following conditions have been met: the student receives one dose of the Hepatitis A vaccine, and the difference between the first dose and the effective date is less than 6 months with a 1 day tolerance.

Pneumococcal PPV

Rule	Requirements
Age 0-1 months No Requirement	Students 1 month old and younger are not required to receive any doses of the PPV vaccine.
Ages 5 and older No Requirement	Students 60 months (5 years) old and older are not required to receive any doses of the PPV vaccine.
1 dose required ages 2-3 months Compliant	Students are considered compliant if the following conditions have been met: the student is as least 2 months, but no older than 3 months old, the student receives one dose of the PPV vaccine, and the difference between the last dose and the student's birthdate is greater than 42 days with a 4 day tolerance.
2 doses required age 4-5 months Compliant	Students are considered compliant if the following conditions have been met: the student is as least 4 months, but no older than 5 months old, the student receives two doses of the PPV vaccine, the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, and the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.

3 doses 12 months Compliant	Students are considered compliant if the following conditions have been met: the student is as least 12 months old, the student receives three doses of the PPV vaccine, the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, the difference between the third and previous dose is greater than 56 days with a 4 day tolerance, and the difference between the first dose and the student's birthdate is less than 11 months with a 4 day tolerance.
2 doses 12 months Compliant	Students are considered compliant if the following conditions have been met: the student is as least 12 months old, the student receives two doses of the PPV vaccine, the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and the difference between the second and previous dose is greater than 56 days with a 4 day tolerance.
1 dose over 24 months Compliant	Students are considered compliant if the following conditions have been met: the student receives one dose of the PPV vaccine, and the difference between the first dose and the student's birthdate is greater than 24 months with a 4 day tolerance.
In-Progress In-Progress	Students are considered in progress if the following conditions have been met: the student receives one dose of the PPV vaccine, and the difference between the first dose and the effective date is less than 28 days with a 0 (zero) day tolerance.
