The Individual Education Program (IEP) or Evaluation team uses the Evaluation to document the student's educational needs and the student's determination of eligibility for special education.

The current format of this document is the MA ESR 2022. Evaluation formats are selected in Eval Types.

 Evaluation Form 

An evaluation must be created whether or not the student is determined to have a disability. A copy of the evaluation team report, including the documentation of eligibility, is given to the student's parents by the school district.

Evaluation Summary Report Editors

The following table lists the editors available on the student's Evaluation, the section(s) of the print format that include the entered information, a Description of what the editor is used for and any special considerations and instructions for using the editor.

Evaluation Information (ADM 1)
Meeting InformationThe Evaluation Header editor lists general information about the Evaluation, including relevant dates.

This editor must be saved before continuing to other parts of the evaluation.


Meeting Date will auto-populate with the current date.

Student Demographics (ADM 1)
Student Information, Assigned School InformationThe Student Demographics editor includes student data entered in the Demographics tool.Clicking Refresh Student Information will synchronize the information in the editor with the current information entered on the Demographics, Households, School and Enrollments tools.
Parent/Guardian Demographics (ADM 1)
Parent/Guardian InformationThe Parent/Guardian Demographics editor populates based on established student/guardian relationships created on the student's Relationships tool or indicated by the guardian checkbox on the Households tool. The editor includes Demographics information for the student's guardian.Clicking Refresh Guardian Information will synchronize information in the editor with the most recent information from the student's guardian's Demographics and Households tool.
SLD: Component 1 (SLD 1)
Specific Learning Disability 1The SLD: Component 1 editor includes a review of the student's previous education, Participation Skills assessed, Performance History considered, and relevant Medical Information.N/A
SLD: Component 2 (SLD 2)
Specific Learning Disability 2The SLD: Component 2 editor describes the method used to determine eligibility.N/A
SLD: Component 3-4 (SLD 3 & 28M/10)
Specific Learning Disability Exclusionary Factors and Specific Learning Disability ObservationThe SLD: Component 3-4 editor includes other considerations that should be addressed when determining eligibility.N/A
SLD: Determination (Flow Chart)
Specific Learning DisabilityThe SLD: Determination editor indicates if the team has identified the student as eligible for special education services as a result of the observations of the previous editors.N/A
Eligibility Determination (Flow Chart)
Special Education Eligibility/Initial and Reevaluation DeterminationThe Eligibility Determination expands on the student's eligibility, including identifying the student's primary disability and the date of determination.N/A
Team Meeting (N 3A)
Attendance SheetThe Team Meeting editor lists the team members who were in attendance for this evaluation and whether those members agreed with the decision reached.

Team members must be added in the Team Members tab before entering participants.


The Print in Eval checkbox must be marked for the team meeting to appear on the printed document.

Consent (N1A)Evaluation Consent FormThe Consent editor indicates the assessment tools and strategies used to gather the information that determines the educational needs of the student.The Parent Response and Parent Input sections only appear in the print format.
