Health vaccines and their associated compliance listings are based on state guidelines. While Infinite Campus maintains standard guidelines for all states, districts have the ability to enter additional compliances. The following information describes vaccine compliance rules as defined by the Hawaii State Department of Health.

DTaP

Rule	Requirements
1 dose 3-4 months Compliant	Students are considered compliant if the following conditions have been met: the student is at least 3 months and no older than 4 months old, the student receives 1 dose of the DT or DTap/DTP vaccine, and the difference between the first dose and the student's birthdate is greater than or equal to 42 days with a 4 day tolerance.
2 doses 5-6 months Compliant	Students are considered compliant if the following conditions have been met: the student is at least 5 months and no older than 6 months old, the student receives 2 doses of the DT or DTap/DTP vaccine, the difference between the first dose and the student's birthdate is greater than or equal to 42 days with a 4 day tolerance, and the difference between the second and previous dose is greater than or equal to 28 days with a 4 day tolerance.
3 doses 7-18 months Compliant	Students are considered compliant if the following conditions have been met: the student is at least 7 months and no older than 18 months old, the student receives 3 doses of the DT or DTap/DTP vaccine, the difference between the first dose and the student's birthdate is greater than or equal to 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than or equal to 28 days with a 4 day tolerance, and the difference between the third and previous dose is greater than or equal to 28 days with a 4 day tolerance.
4 doses under 4 Compliant	Students are considered compliant if the following conditions have been met: the student is no older than 47 months old, the student receives 4 doses of the DT or DTap/DTP vaccine, the difference between the first dose and the student's birthdate is greater than or equal to 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than or equal to 28 days with a 4 day tolerance, the difference between the third and previous dose is greater than or equal to 28 days with a 4 day tolerance, the difference between the fourth and previous dose is greater than or equal to 6 months with a 4 day tolerance, and the difference between the fourth dose and the student's birthdate is greater than or equal to 12 months with a 4 day tolerance.
4 doses over 4 Compliant	Students are considered compliant if the following conditions have been met: the student is at least 48 months old, the student is in state grade: 94: Pre-Kindergarten (Age 2), 93: Pre-Kindergarten (Age 3), 92: Pre-Kindergarten (Age 4), 91: Kindergarten, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12, 31: SpEd-Overage, 32: SpEd-Overage 19, 33: SpEd-Overage 20, 34: SpEd-Overage 21, the student receives 4 doses of the DT or DTap/DTP vaccine, the difference between the first dose and the student's birthdate is greater than or equal to 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than or equal to 28 days with a 4 day tolerance, the difference between the third and previous dose is greater than or equal to 28 days with a 4 day tolerance, the difference between the fourth and previous dose is greater than or equal to 6 months with a 4 day tolerance, and the difference between the fourth dose and the student's birthdate is greater than or equal to 48 months with a 4 day tolerance.
5 doses Compliant	Students are considered compliant if the following conditions have been met: the student is at least 48 months old, the student is in state grade: 94: Pre-Kindergarten (Age 2), 93: Pre-Kindergarten (Age 3), 92: Pre-Kindergarten (Age 4), 91: Kindergarten, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12, 31: SpEd-Overage, 32: SpEd-Overage 19, 33: SpEd-Overage 20, 34: SpEd-Overage 21, the student receives 5 doses of the DT or DTap/DTP vaccine, the difference between the first dose and the student's birthdate is greater than or equal to 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than or equal to 28 days with a 4 day tolerance, the difference between the third and previous dose is greater than or equal to 28 days with a 4 day tolerance, the difference between the fourth and previous dose is greater than or equal to 6 months with a 4 day tolerance, the difference between the fourth dose and the student's birthdate is greater than or equal to 12 months with a 4 day tolerance, the difference between the fifth and previous dose is greater than or equal to 6 months with a 4 day tolerance, and the difference between the fifth dose and the student's birthdate is greater than or equal to 48 months with a 4 day tolerance.
3 doses after 7 years Compliant	Students are considered compliant if the following conditions have been met: the student is at least 84 months old, the student is in state grade: 91: Kindergarten, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12, 31: SpEd-Overage, 32: SpEd-Overage 19, 33: SpEd-Overage 20, 34: SpEd-Overage 21, the student receives 3 doses of the DT or DTap/DTP vaccine, the difference between the first dose and the student's birthdate is greater than or equal to 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than or equal to 28 days with a 4 day tolerance, and the difference between the third and previous dose is greater than or equal to 6 months with a 4 day tolerance AND the student receives one dose of the Tdap vaccine, the difference between the dose and the student's birthdate is greater than or equal to 84 months with a 4 day tolerance.
1 dose in Progress In Progress	Students are considered in progress if the following conditions have been met: the student receives 1 dose of the DT or DTap/DTP vaccine, and the difference between the dose and the effective date is less than 28 days with a 0 (zero) day tolerance.
2 doses in Progress In Progress	Students are considered in progress if the following conditions have been met: the student receives 2 doses of the DT or DTap/DTP vaccine, and the difference between the last dose and the effective date is less than 6 months with a 1 day tolerance.
3 doses in Progress In Progress	Students are considered in progress if the following conditions have been met: the student receives 3 doses of the DT or DTap/DTP vaccine, and the difference between the last dose and the effective date is less than 6 months with a 1 day tolerance.

Polio

Rule	Requirements
0-1 month No Requirement	Students 1 month old and younger are not required to receive any doses of the Polio vaccine.
1 dose 2-4 months Compliant	Students are considered compliant if the following conditions have been met: the student is at least 2 months, but no older than 4 months old, the student receives 1 dose of the IPV, OPV, or IPV/OPV combined Polio vaccine, and the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance.
2 doses 5-18 months Compliant	Students are considered compliant if the following conditions have been met: the student is at least 5 months, but no older than 18 months old, the student receives 2 doses of the IPV, OPV, or IPV/OPV combined Polio vaccine, the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, and the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.
3 doses 19 months Compliant	Students are considered compliant if the following conditions have been met: the student is at least 19 months old, the student is in state grade: 94: Pre-Kindergarten (Age 2), 93: Pre-Kindergarten (Age 3), 92: Pre-Kindergarten (Age 4), the student receives 3 doses of the IPV, OPV, or IPV/OPV combined Polio vaccine, the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, and the difference between the third and previous dose is greater than 28 days with a 4 day tolerance.
3 doses after 4 Compliant	Students are considered compliant if the following conditions have been met: the student is at least 48 months old, the student receives 3 doses of the IPV, OPV, or IPV/OPV combined Polio vaccine, the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, the difference between the third and previous dose is greater than 6 months with a 4 day tolerance, and the difference between the third dose and the student's birthdate is greater than 48 months with a 4 day tolerance.

Students Enrolled Before 07/01/2020 4 doses Compliant	Students are considered compliant if the following conditions have been met: the student is enrolled on 06/30/2020 or before, the student receives 4 doses of the IPV, OPV, or IPV/OPV combined Polio vaccine, the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, the difference between the third and previous dose is greater than 28 days with a 4 day tolerance, and the difference between the fourth and previous dose is greater than 28 days with a 4 day tolerance.
1 dose in Progress In Progress	Students are considered in progress if the following conditions have been met: the student receives 1 dose of the IPV, OPV, or IPV/OPV combined Polio vaccine, and the difference between the dose and the effective date is less than 28 days with a 0 (zero) day tolerance.
2 doses in Progress In Progress	Students are considered in progress if the following conditions have been met: the student receives 2 doses of the IPV, OPV, or IPV/OPV combined Polio vaccine, and the difference between the last dose and the effective date is less than 6 months with a 1 day tolerance.

MMR

Rule	Requirements
0-15 months No Requirements	Students 0 - 15 months old are not required to receive any doses of the MMR vaccine.
1 dose PK Compliant	Students are considered compliant if the following conditions have been met: the student is at least 16 months old, the student is in state grade: 94: Pre-Kindergarten (Age 2), 93: Pre-Kindergarten (Age 3), 92: Pre-Kindergarten (Age 4), the student receives one dose of the MMR or MMRV vaccine, and the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance.
2 doses K-12 Compliant	Students are considered compliant if the following conditions have been met: the student receives two doses of the MMR or MMRV vaccine, the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.
In Progress In Progress	Students are considered in progress if the following conditions have been met: the student receives one dose of the MMR or MMRV vaccine, and the difference between the last dose and the effective date is less than 28 days with a 0 (zero) day tolerance.

Hepatitis B

Rule	Requirements
1 dose 0-1 month Compliant	Students are considered compliant if the following conditions have been met: the student is between 0-1 month old, and the student receives one dose of the Hepatitis B vaccine.
2 doses 2-18 months Compliant	Students are considered compliant if the following conditions have been met: the student is at least 2 months, but no older than 18 months old, the student receives two doses of the Hepatitis B vaccine, and the difference between the second and previous dose is greater than or equal to 28 days with a 4 day tolerance.
3 doses Compliant	Students are considered compliant if the following conditions have been met: the student receives three doses of the Hepatitis B or Hepatitis B – 3 Dose vaccine, the difference between the second and previous dose is greater than or equal to 28 days with a 4 day tolerance, the difference between the third and second valid dose is greater than or equal to 56 days with a 4 day tolerance, the difference between the third and first valid dose is greater than or equal to 112 days with a 4 day tolerance, and the difference between the third dose and the student's birthdate is greater than or equal to 168 days with a 4 day tolerance.
2 doses In Progress In Progress	Students are considered in progress if the following conditions have been met: the student receives two doses of the Hepatitis B vaccine, and the difference between the second dose and the effective date is less than 56 days with a 0 (zero) day tolerance.
1 dose In Progress In Progress	Students are considered in progress if the following conditions have been met: the student receives one dose of the Hepatitis B vaccine, and the difference between the second dose and the effective date is less than 28 days with a 0 (zero) day tolerance.

Hib

Rule	Requirements
Age 0-1 month No Requirements	Students age 0-1 month old are not required to receive any doses of the Haemophilus influenza, type B (Hib) vaccine.
Age 5 and older No Requirements	Students 60 months old and older are not required to receive any doses of the Haemophilus influenza, type B (Hib) vaccine.
1 dose Age 2-3 months Compliant	Students are considered compliant if the following conditions have been met: the student is at least 2 months, but no older than 3 months old, the student receives one dose of the Haemophilus influenza, type B (Hib) or Hib 3-dose series (Hib-3) vaccine, and the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance.
2 doses Age 4-5 months Compliant	Students are considered compliant if the following conditions have been met: the student is at least 4 months, but no older than 5 months old, the student receives two dose of the Haemophilus influenza, type B (Hib) or Hib 3-dose series (Hib-3) vaccine, the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, and the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.

1 dose In Progress after 1 yr In Progress	Students are considered in progress if the following conditions have been met: the student receives one dose Haemophilus influenza, type B (Hib) or Hib 3-dose series (Hib-3) vaccine, the difference between the last dose and the student's birthdate is greater than 12 months with a 0 (zero) day tolerance, and the difference between the last dose and the effective date is less than 56 days with a 0 (zero) day tolerance.

Varicella

Rule	Requirements
0-11 months No Requirement	Students age 11 months old and younger are not required to receive any doses of the Varicella vaccine.
1 dose PK Compliant	Students are considered compliant if the following conditions have been met: the student is in state grade: 94: Pre-Kindergarten (Age 2), 93: Pre-Kindergarten (Age 3), 92: Pre-Kindergarten (Age 4), the student receives one dose of the Varicella or MMRV vaccine, and the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance.
2 doses K-12 Compliant	Students are considered compliant if the following conditions have been met: the student receives two doses of the Varicella or MMRV vaccine, the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.
1 Dose K-12 Compliant	Students are considered compliant if the following conditions have been met: the student Enrollment End Date is before 04/15/2019, the student receives one dose of the Varicella or MMRV vaccine, the student is in state grade(s): 91, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12, and the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance.
1 dose <13 yo In Progress	Students are considered in progress if the following conditions have been met: the student is no older than 155 months old, the student receives one dose of the Varicella or MMRV vaccine, and the difference between the last dose and the effective date is greater than 84 days with a 0 (zero) day tolerance.
1 dose after 13+ In Progress	Students are considered in progress if the following conditions have been met: the student is at least 156 months old, the student receives one dose of the Varicella or MMRV vaccine, and the difference between the last dose and the effective date is greater than28 days with a 0 (zero) day tolerance.

Hepatitis A

Rule	Requirements
0-18 months No Requirement	Students age 18 months old and younger are not required to receive any doses of the Hepatitis A vaccine.
Students Enrolled Before 07/01/2020 No Requirement	Students enrolled on 6/30/2020 and before are not required to receive any doses of the Hepatitis A vaccine.
2 doses Compliant	Students are considered compliant if the following conditions have been met: the student receives two doses Hepatitis A vaccine, the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and the difference between the second and previous dose is greater than 6 months with a 4 day tolerance.
1 dose In Progress	Students are considered in progress if the following conditions have been met: the student receives one dose Hepatitis A vaccine, and the difference between the last dose and the effective date is less than 6 months with a 1 day tolerance.

Tdap

Rule	Requirements
1 dose over 10 yo Grades 7-12 Compliant	Students are considered compliant if the following conditions have been met: the student is at least 120 months old, the student is in state grade: 07, 08, 09, 10, 11, 12, the student receives one dose of the Tdap vaccine, and the difference between the last dose and the student's birthdate is greater than 120 months with a 4 day tolerance.
Grades 7-8 21-22 Non-compliant	Students are considered non-compliant if the following conditions have been met: the student is at least 120 months old, the student is in state grade: 07, 08, and the student has not received any doses of the Tdap vaccine.
Grades 7-9 22-23 Non-compliant Active Start Date: 7/1/22	Students are considered non-compliant if the following conditions have been met: the student is at least 120 months old, the student is in state grade: 07, 08, 09, and the student has not received any doses of the Tdap vaccine.
Grades 7-10 23-24 Non-compliant Active Start Date: 7/1/23	Students are considered non-compliant if the following conditions have been met: the student is at least 120 months old, the student is in state grade: 07, 08, 09, 10, and the student has not received any doses of the Tdap vaccine.
Grades 7-11 24-25 Non-compliant Active Start Date: 7/1/24	Students are considered non-compliant if the following conditions have been met: the student is at least 120 months old, the student is in state grade: 07, 08, 09, 10, 11, and the student has not received any doses of the Tdap vaccine.
Grades 7-12 25-26 Non-compliant Active Start Date: 7/1/25	Students are considered non-compliant if the following conditions have been met: the student is at least 120 months old, the student is in state grade: 07, 08, 09, 10, 11, 12, and the student has not received any doses of the Tdap vaccine.
Enrolled After 8/3/2020 Non-compliant	Students are considered non-compliant if the following conditions have been met: the student is at least 120 months old, the student is in state grade: 07, 08, 09, 10, 11, 12, and the student's enrollment start date is after 8/4/2020.

Meningococcal MCV4

Rule	Requirements
1 dose over 10 yo Grades 7-12 Compliant	Students are considered compliant if the following conditions have been met: the student is at least 120 months old, the student is in state grade: 07, 08, 09, 10, 11, 12, the student receives one dose of the Meningococcal vaccine, and the difference between the last dose and the student's birthdate is greater than 120 months with a 4 day tolerance.
Grades 7-8 21-22 Non-compliant	Students are considered non-compliant if the following conditions have been met: the student is at least 120 months old, the student is in state grade: 07, 08, and the student has not received any doses of the Meningococcal vaccine.
Grades 7-9 22-23 Non-compliant Active Start Date: 7/1/22	Students are considered non-compliant if the following conditions have been met: the student is at least 120 months old, the student is in state grade: 07, 08, 09, and the student has not received any doses of the Meningococcal vaccine.
Grades 7-10 23-24 Non-compliant Active Start Date: 7/1/23	Students are considered non-compliant if the following conditions have been met: the student is at least 120 months old, the student is in state grade: 07, 08, 09, 10, and the student has not received any doses of the Meningococcal vaccine.
Grades 7-11 24-25 Non-compliant Active Start Date: 7/1/24	Students are considered non-compliant if the following conditions have been met: the student is at least 120 months old, the student is in state grade: 07, 08, 09, 10, 11, and the student has not received any doses of the Meningococcal vaccine.
Grades 7-12 25-26 Non-compliant Active Start Date: 7/1/25	Students are considered non-compliant if the following conditions have been met: the student is at least 120 months old, the student is in state grade: 07, 08, 09, 10, 11, 12, and the student has not received any doses of the Meningococcal vaccine.
Enrolled after 8/3/2020 No Requirement	Students whose Enrollment Start Date is after 8/4/2020 are not required to receive any doses of the MCV4 vaccine.

Human Papillomavirus HPV

Rule	Requirements

Grades 7-9 22-23 Non-compliant Active Start Date: 7/1/22	Students are considered non-compliant if the following conditions have been met: the student is at least 108 months old, the student is in state grade: 07, 08, 09, and the student has not received the required doses of the HPV vaccine.
Grades 7-10 23-24 Non-compliant Active Start Date: 7/1/23	Students are considered non-compliant if the following conditions have been met: the student is at least 108 months old, the student is in state grade: 07, 08, 09,10, and the student has not received the required doses of the HPV vaccine.
Grades 7-11 24-25 Non-compliant Active Start Date: 7/1/24	Students are considered non-compliant if the following conditions have been met: the student is at least 108 months old, the student is in state grade: 07, 08, 09,10, 11, and the student has not received the required doses of the HPV vaccine.
